The travel form must be completed and returned to the surgery at least 6 weeks before you travel.
- If you require any vaccinations relating to foreign travel you must first visit the NHS website fit for travel. It is important to make this initial visit to the website as it will guide you through the areas you are visiting and give you some idea of which vaccinations may be necessary for your trip.
- After visiting the website please read and complete our travel questionnaire. Traveller Questionnaire
- Once you have fully completed the questionnaire and the surgery have received it a nurse will look at your travel details and contact you within two weeks, to either make an appointment for vaccinations or advise you that you do not need any vaccinations on your trip.
Some travel vaccination is not covered by the NHS and incurs charges. Please see the current price list attached to the travel questionnaire for details.
