Practice Boundary

We are writing to all our registered patients who are living outside of the Practice boundary to advice that we will not be able to continue providing general medical services as your General Practitioners. As the new GP Partners at Brailsford and Hulland Medical Practice, we have reviewed our registered Practice list and note that we have patients  currently living outside of our designated Practice area.

We have an exceptionally large geographical area within our designated practice boundary and aligned to this is our extended community team which is managed by Derbyshire Community Health Services, including district nurses and home therapy teams. These aligned teams work together to provide health care within our practice area. This change is to ensure we continue to provide the highest standards of care to patients within our practice area in close cooperation with our extended community team who are only allowed to visit patients within our designated practice area.

“The area in respect of which persons resident in it will, subject to any other terms of the Contract relating to patient registration, be entitled to register with the Contractor, or seek acceptance by the Contractor as a temporary resident, is the area surrounding Brailsford and Hulland Ward, bordered by the villages of Boylestone, Brassington, Carsington, Church Broughton, Dalbury, Idridgehay, Kedleston, Kirk Ireton, Kniveton, Mackworth Village, Osmaston, Sutton on the Hill, Turnditch, Windley, Wyaston and Yeaveley[1]. “

Practice Notice

Closed for staff training

11 June 2021 from 13:00

The pracitce will be closed for staff training from 13:00 on Friday 11 June and will reopen as normal on Monday 14 June 2021.

During this time the prescription collection window will remain open.
