You can order you repeat orders by :-
Ticking and returning the white reorder slip (right hand side example below) to the dispensary or post through the letterbox at the practice (right hand side of the building).
This slip will be attached to your last bag of collected medication or should have been returned to you by the pharmacy if you had it dispensed elsewhere.
Please allow 5-7 days for reorders.
You may also hand write/type a note. Please be sure to include name, date of birth, first line of you address and what medication you require.
